Welcome to the Emergency Care Technician (ECT) CPG Transition Course! The HPCSA released the new Clinical Practice Guidelines in 2018 and you have taken the first step in aligning yourself with the new guidelines. Please work through all the modules thoroughly before challenging the module examination. You need 80% for each module examination to proceed to the next module. Each module has learning units. For any enquiries on the content, please e-mail education@phec.co.za
As an ECT practitioner you gained new skills and controversially also lost a few skills. PHEC saw it fit to refresh some skills which may relate to the change in scope of practice, please note that you will not be assessed on the revision content. 1MODULE 1 – OXYGEN AND VENTILATION ECT
OBJECTIVES
On completion of this module, the learner will be able to
- Apply appropriate strategies of airway support with the use Non-invasive ventilation techniques
- Demonstrate effective techniques using appropriate technology and evidence for best practice of airway maintenance at ECT scope of practice according to CPG July 2018

2SpO2 review
LEARNING UNIT 1
In this Learning Unit we will be revising some theory pertaining to blood saturation measurement theory. Read through the material.

3ETCO2 Use
LEARNING UNIT 2
In this Learning Unit we will be looking at the use of Capnography. You as an ECT practitioner will be able to assess your patients’ Capnography using Nasal Cannula devices. Although the HPCSA excluded the use of capnography/capnometry via extra-glottic devices on your scope of practice, the same principles apply. Read through the material and watch the video.

4Use of peak flow measurement

5Supraglottic review
LEARNING UNIT 4
You as an ECT practitioner is the lowest qualified member of the team that can secure an advanced airway on a live patient. ECA practitioners may only secure an advanced airway in the event of cardiac arrest victims. Read through the material and watch the videos.

6Surgical Cricothyroidotomy
LEARNING UNIT 5
In this Learning Unit we will be looking at the Surgical Cricothyroidotomy, which is a newly added skill to your scope of practice. This is also the skill that will need to be signed off at one of our Sign-Off Sites. Read through the material and watch the video.

7Use of Non-Invasive ventilation, Oxygen Driven without Mechanical Ventilator

In this Learning Unit we will be looking at the application of Non-Invasive Ventilation using Oxygen Driven devices. This is also the skill that will need to be signed off at one of our Sign-Off Sites. Read through the material and watch the video.

11MODULE 2 – CIRCULATORY MANAGEMENT
So now that we have airway and breathing sorted, let’s have a look at the circulatory system. Welcome to Module 2 and congratulations on passing Module 1. Trauma management forms a big part in this module, and we recommend that you do some other bleeding control and trauma management courses available for pre-hospital providers. Bleeding control is a basic skill, but in some situations, it may be a very difficult task. We will also be looking at intravenous infusion with the use of volumetric infusion pumps and syringe drivers. 12Control external haemorrhage with haemostatic agents
LEARNING UNIT 1
Circulation is vital for all human beings thus as an emergency care provider it is important to be knowledgeable of all forms of restoration of circulation to one’s patients. In this Learning Unit we have included revision statements pertaining to perfusion. Topical haemostatic agents have evolved into various classes over the years, the HPCSA have released in the latest guidelines that this is one of the new skills that you as an ECT should be competent with in practice. This learning unit will guide you using topical haemostatic agents. Read through the material and watch the video.

13Control external haemorrhage with the safe use of a tourniquet
LEARNING UNIT 2
In this Learning Unit we will be looking at the use of the tourniquet to stop bleeding. As an ECT you need to familiarize yourself with the identification of a life-threatening bleed which may need to be controlled with the use of this device. There are various types of tourniquets which you will learn about in this Learning Unit. Read through the material and watch the videos.

14Precordial Thump and its place during resuscitation
LEARNING UNIT 3
There is a lot of controversy around the performance of the precordial thump, however its use is outlined in the new CPG and in this learning unit we cover the indications and performance of the skill. Read through the material in this Learning Unit and watch the video.

15Use of the Non-Pneumatic Anti-Shock Garment (NASG)
LEARNING UNIT 4
In this learning unit we will be looking at the use of the non-pneumatic anti-shock garment (NASG). In the Obstetrics Module, the use of the NASG will be addressed more thoroughly. Read through the material in this learning unit and watch the video.

16Infusion pump and syringe driver
LEARNING UNIT 5
In this Learning Unit we will be looking at the use of Infusion Devices. This is also the skill that will need to be signed off at one of our Sign-Off Sites. Please note that you will receive frequently used infusion calculations in Module 4 – Pharmacology in your downloadable Drug book and a document which explains drug calculations, but you will have to decide which concentrations works best for you. 20Recognition and Management Obstetric of Emergencies

22Umbilical vein catheterisation
LEARNING UNIT 3
Umbilical Vein Cannulation (UVC) is not a skill that is commonly performed in the pre-hospital environment. To maintain proficiency with this skill, we recommend that you as a practitioner should work additional shifts in neonatal units in hospitals and to stay current with the latest practices. 23Safe use of an Incubator and thermal control

Incubators are commonly used in the obstetric transfer scenarios; in this Learning Unit we will go over the emergent use of rewarming strategies in the obstetrics environment and also cover the use of incubators.
